General Description of the Feature

in the Lead module when a Lead is in state Order the user can select to 'Order Customer Concerns'

In the pop up the user can select estimation date and customer concerns

User can add Estimated Completion Date and Customer Concern. After saving the transition a request has been created from DSW to DMS with all the information about the customer concerns and with the Estimated Completion Date 
 

Steps to Reproduce

  1. Login to app (https://sit-proaxia-vss5.onedealer.com/ - ezwardi.tengku@proaxia-consulting.com/123!)
  2. Create a lead
  3. Select Create Offer -> Create Order -> Order Customer Concerns
  4. Select estimation date and CCs
  5. Click on save
